Job Title:                                        Harm Reduction Case Worker, Open Access Service

 

Responsible to:                             Head of Day Services/ Day Services Manager

 

Responsible for:                           Assessment and case management of clients referred to the MQI Case Management Service

Reports to:                                    Crisis and Case Management Co-Ordinator

 

Objective:                                      To work directly with clients in developing care plans using a case   management approach and delivering harm reduction interventions.

To facilitate low threshold community detoxification in conjunction with partner agencies

To ensure high quality of service is upheld in line with best standards and MQI policies and procedures.

Main Duties/Responsibilities

  • Provide one-to-one casework and support to clients referred to the detox service, developing care plans and carrying out needs and risk assessments with clients as required.
  • Case management and care planning, undertaking interventions as part of a care plan, attending case reviews/case management meetings in partnership with our clients, other disciplines, and agencies.
  • Facilitating low threshold community detoxification in conjunction with the client, GP’s, accommodation services and partner agencies.
  • Work with partner agencies to develop strong professional relationships, evaluating and developing best practice, policies and procedures in conjunction with the service Coordinator.
  • To work both within the Open Access Service and offsite to deliver a high quality of service and ensure clear communication between partner agencies.
  • Assessing clients who are referred to the Community Detox Program, and collaborating with partner agencies, GP services and case management team to evaluate each case and decide upon appropriate progression routes.
  • Working with clients during their detox to develop move on options and linking them with appropriate services post detox, referring clients to appropriate accommodation, welfare, treatment, and support services as needed.
  • To facilitate psycho-educational and health promotional groups as part of the detoxification programme to ensure clients receive holistic and comprehensive support as part of their residential programme.
  • To undertake and lead new projects related to the Drug Services as assigned by the Programmes and Drug Services Co-Ordinator and/or the Head of Day Services.
  • Ensure that a high quality of service is upheld and delivered to clients.
  • Work with all clients and win their trust and respect and deal with difficult or problematic situations in a sensitive manner within the framework of the organisation’s policies and record such incidents as appropriate.
  • Carry out administrative procedures in accordance with organisation policies.
  • To be involved in the preparation of reports, statistics and other information that may be required by the Crisis and Case Management Co-Ordinator and/or the Service Manager.
  • Attend internal/external meetings and training as required by the Crisis and Case Co-Ordinator.
  • Have a flexible approach to the work in response to organisational change, development and review of best practice.
  • To work as part of the team; engage with other team members, support colleagues, participate in meetings as required, and be open to reasonable requests from the Crisis and Case Management Co-Ordinator/Service Manager to ensure effective working relationships.
  • Work within the policies of MQI, both organisation wide and project specific in order to ensure that a consistent delivery of service and quality standards are adhered to in the project.
  • Share responsibility to work cohesively where applicable with CE Participants and Volunteers for the delivery of this service and to act as a mentor to those who are working/volunteering in the service.
  • Report to the Crisis & Case management Service Co-Ordinator for individual support, supervision, performance appraisal working within the parameters of the MQI Quads, HR and H&S policies and procedures.
  • Ensure compliance with organisation policies to ensure consistent delivery of services and maintenance of quality standards including:
    • Fire
    • Health and Safety
    • Confidentiality
    • Needs assessment, key working, care planning
    • Boundaries
    • Quads
  • Undertake any other duties as may be assigned from time to time.

Person Specification

The ideal candidate for this post will have:

  • A relevant third level qualification in the social care field and at least 2 years’ experience working in a low threshold service
  • An experience of working with issues relating to care planning and case management regarding alcohol and drug misuse, specifically within a low threshold environment and an awareness of behaviour and social problems affecting them
  • Experience and understanding of undertaking risk assessments, assessing needs and developing and following through on care plans
  • Possess strong interpersonal skills and have experience of working in a multidisciplinary/multiagency team within a low threshold environment
  • Ability to communicate and engage challenging behaviour in a respectful and dynamic way
  • Experience and understanding of issues relating to drug and alcohol use/homelessness / mental health and undertaking harm reduction interventions as required
  • Strong knowledge of homeless and drug services and relevant referral pathways is a distinct advantage
  • Possess a flexible professional approach to their work, and to be open to working in a change environment.
